To perform this jobsuccessfully, an individual must be able to satisfactorily perform eachessential function listed below.
·Provides 24/7coverage for all assigned clients/families.
·Providesassessments in the areas of mental status, personality, behavioral responsepatterns, relationships, and family interactions.
·Proposes aservice or treatment plan based on assessment of needs and makes practicalsuggestions for change.
·Provides activityand participation based interventions and approaches developed from trainingmaterials, experience, families’ successes, and peers.
·Makes referralsto services and resources on behalf of families.
·Builds andmaintains positive relationships with personnel from referring/ collateralagencies. Provides guidance for the family in their interaction with referringagencies, and other community organizations as needed.
·Assures thatdocumentation and records are kept confidential, organized, and complete andsubmitted to supervisor in an accurate and timely manner.
·Participates inteam meetings with peers and in collateral team meeting when needed. Supportspeer development by addressing areas of concern for professional developmentand team performance issues.
·Provides aclosure and aftercare plan for each family/client.
·May providetraining or field supervision of new or tenured peers; review and edit peerrecords and documentation, marketing and education presentations; training anddevelopment for the region or team meetings,
·Performs otherrelated duties and activities as required.
